Repositioning POLST for Clarity, Access and Impact

The National POLST Collaborative partnered with Landslide Creative to reimagine their website as the digital home for a growing national movement focused on improving serious illness care. As POLST clarified its mission, vision, and long-term sustainability goals, the organization needed a website that reflected its leadership role, supported state programs, and engaged both clinicians and the public with greater clarity.

Through discovery and strategic planning, we identified a key challenge: too much emphasis on advance care planning language was creating confusion and disengagement. Guided by evidence-based serious illness messaging principles that prioritize choice, benefits, and accessible language, we restructured the site to simplify the user journey and present information in a clear, linear format that keeps visitors oriented. We also elevated membership pathways and national collaboration priorities to align with POLST’s evolving organizational strategy.

The new website strengthens National POLST’s presence while making portable medical orders easier to understand and implement across care settings.
